I understand that karate is a contact sport. As such, I am aware that I am participating in an activity that involves physical contact and injuries may occur, which may potentially involve great bodily harm or even death and I do so at my own risk. I waive any claim or cause of action I may have against the United States Karate Alliance, Inc., its directors, officers, shareholders, officials, judges, seminar instructors, other competitors, and the Arizona Grand Resort and Spa from any and all damages, claims, or lawsuits resulting from either injury to myself, or that may be caused by others, from my participation in the Alliance Nationals Tournament or any of the Alliance Seminars. I state that I do not suffer from any physical and/or mental conditions which may affect my participation in this tournament. I further agree to conduct myself in a sportsmanlike manner and understand that if I fail to do so, I may be disqualified from the tournament and not entitled to a refund. The undersigned (or undersigned’s legal guardian) agrees that any photos or videos taken of the undersigned during this event may be used by the United States Karate Alliance in its promotional materials or on its website with no compensation or other remuneration due to the undersigned and the undersigned hereby waives any claims they may have against the United States Karate Alliance for the use of such photos or videos.
